This survey is designed to collect feedback from separating employees about their working life at Gold Coast Health and reasons for leaving.

It is our vision that every staff member’s experience working with GCH is a positive one. Your feedback allows us to bring this vision to life – through ongoing improvements.

All questions in this survey are optional. Most responses will be combined with those from other respondents to provide anonymity before being shared with management. Free text responses will only be visible to relevant divisional executives. Themes may also be provided to relevant business areas for consideration and action, such as diversity and inclusion.

Queensland Health respects your privacy and is committed to protecting your personal information. Any personal information you provide to us through this service will be managed within the requirements of the Information Privacy Act 2009.
